EVALUATION DU TRAITEMENT ORTHOPEDIQUE DES FRACTURES EXTRA-ARTICULAIRES DE L’EXTREMITE DISTALE DU RADIUS A BASCULE POSTERIEURE

Introduction:

L’objectif de cette étude était d’évaluer les resultats fonctionnels du traitement orthopédique des fractures de l’EDR à bascule postérieure.

Matériels et méthodes:

Il s’agissait d’une étude prospective de type analytique de 6 mois allant 1er janvier au 30 juin 2018, réalisée dans le service d’orthopédie traumatologie du Centre Hospitalier Universitaire Aristide Le Dantec de Dakar.

Etait inclus dans l’étude, tout patient présentant une fracture fermée extra-articulaire de l’EDR à bascule postérieure (fracture de Pouteau-Colles), associée ou non à une fracture de la styloïde ulnaire (fracture de Gérard Marchand) traitée orthopédiquement puis suivi jusqu’à consolidation; n’était pas inclus dans l’étude tout patient dont le dossier était incomplet.

Le traitement orthopédique consistait à faire une réduction et une contention par un plâtre brachio-antébrachio-palmaire (BABP) pendant 21 jours puis remplacé par une manchette plâtrée, le poignet en position neutre pendant 24 jours.

Les patients étaient suivi a partir de Jour (J) 0 du traitement; J1 ; Entre J7,  J15 et J21; J45; J75 dans le but de rechercher un syndrome compartimental, un déplacement secondaire, un cal vicieux et une limitation fonctionnelle.     

Resultats :

Un total de 67 patients ont été colligé (53 fractures de Pouteau-Colles et 14 fractures de  Gérard-Marchant) et traité orthopédiquement avec des resultats fonctionnels au terme du suivi satisfaisants chez 52 des  patients. 

- A J0 nous avons constaté 7 échecs ayant conduit à un traitement chirurgical

- Entre J7-J75 nous avons  constaté 4 perdus de vue

- Entre J21-J75 nous avons observé 8 déplacements secondaires ayant aboutis a des cals vicieux

- Entre  J21-J75 nous avons observé 48 consolidations en bonne position.

Conclusion:

Le traitement orthopédique  des fractures de l’EDR a bascule postérieure a permis dans notre étude une une prise en charge non agressive, rapide à moindre cout avec des resultats satisfaisants.

Abstract:

Introduction:

The purpose of this study was to evaluate the functional results of orthopaedical management of distal radial extremity fractures posteriorly displaced.

Materials and Methods: It was a prospective, analytical study performed within 6 months from 1^st January to 30^th of June in the traumatological and orthopaedical department of the university hospital Aristide Le Dantec of Dakar.

Was included in this study any patient with extra-articular fracture of DRE posteriorly displaced (Pouteau-Colles fracture) associated or not to ulnar styloid process fracture (Gérard-Marchand fracture) orthopaedically managed and followed up till consolidation. Was not included in this study any patient with noncompleted folder.

The orthopaedical management was characterized by closed reduction and contention by brachial antebraquiopalmar plaster for 21 days afterward replaced by plaster cuff with the wrist in neutral position for 24 days.

All the patients were followed up from the zero day (D) of treatment; D1; between D7, D15 and D21; D45; D75 in order to find a compartimental syndrome, a displacement, a vicious callus and functional limitation.

Results:

Sixty seven patients were collected in which 53 pouteau-colles fractures and 14 Gerard-Marchant fractures orthopaedically managed with a satisfactory functional result at the end of the follow up in 52 patients.

At D0 we noticed 7 failures in orthopaedical management which led to surgery;

Between D7-D75 we noticed the loss of patients;

Between D21-D75 we noticed 8 displacement which led to vicious callus;

Between D21-D75 we noticed 48 good consolidations.

Conclusion:

The Orthopaedic management of distal radial extremity fractures posteriorly displaced allowed in our study a less expensive, non-invasive and faster management with satisfactory results.
